No it probably means that when you use it you die seven days before you"re supposed to so even if you teleport I doubt you"d still die instantly somehow

These buttons seem like they have no downside, I could always just not teleport and have the option for if, say I'm about to die or go on a journey longer than 7 days

expectancy is to do with your perception. It doesn't say your life will actually be shorter.
